Our Story
HCH (HealthCare at Home) Primary Care Clinic is a hybrid practice that provides primary and wellness care in our clinic as well as home visits. We believe that healthcare should be centered around your needs, and aim to provide top-class, comprehensive and effective primary care.
HCH Primary Clinic started out providing home-visits for clients that were homebound and had transportation difficulties in 2017. Our practice grew and office visits were added in 2019 to cater to our clients in the surrounding communities.
Our Mission is to educate and provide excellent care to help our clients achieve the highest level of health and wellbeing.
Our clinic utilizes Nurse Practitioners to provide holisitic individulaized care.
Who are Nurse Practitioners?
Nurse Practitioners (NPs) are on the front lines of care, treating patients – in sickness and in health. NPs emphasize healthy living through education, counseling and disease prevention. By treating the whole person instead of isolated health events, NPs work with patients to improve their overall health.
NPs provide advanced primary, acute and specialty care for all types of patients. In every state, NPs assess patients, order and interpret tests, make diagnoses and provide treatment – including prescribing medications.
